I was watching the machine from the ground. My wife had my phone and was adjusting the camera pitch.

Wife and I took a drive today and grabbed some photos and videos. The photos were shot with a Sony NEX 5n and the video was all shot with the P2V+. My wife had control of the camera so I could focus on flying. Given it was her first time, she didn't do too bad.

Make sure the GPS antenna and FC are pointed in the right direction. You should be taking off in GPS mode. Did you do a compass calibration before the flight? For the altitude issue, it could be voltage alarm or that you don't have the FC set for the proper voltage from your battery.

The answer is "it depends". Are you trying to make a super fast sport flyer? Carry a GoPro? Carry an NEX? What is best is determined by the use case. What works great for one person is not the ideal setup for someone else. I can give you a suggestion for 15 minute flight times and then find out...

Define plenty of flight time? You will want extended arms, more torquey motors, and larger props to provide maximum flight time and possible dual batteries.
